Housing Stabilization Services
Housing Stabilization Services (HSS) is a support program designed for individuals who are 18 years or older, have a disability, and are receiving MA (Medical Assistance). If individuals have a waiver, they can be referred to HSS through their case manager. For those without a waiver, they will need to provide a professional statement of need and a person-centered plan that focuses on housing. The housing-focused person-centered plan can be completed by a targeted case manager or care coordinator. In the absence of a targeted case manager or care coordinator, a housing consultant is available to assist in creating the plan.
Life Skills Training and Support
Our life skills training and support services help individuals develop the skills and knowledge necessary to achieve independence and well-being. Our programs include education and training on important life skills, such as budgeting, cooking, and self-care, to help clients lead fulfilling and successful lives.
Health and Wellness Education and Support
Our health and wellness education and support services help clients maintain and improve their physical and mental health. Our programs include health education and wellness coaching, as well as connections to community resources and support systems, to help clients achieve and maintain good health..
Assistance with Accessing Benefits and Entitlements
Our assistance with accessing benefits and entitlements services help clients understand and access the benefits and services they are entitled to. Our case managers work with clients to identify and apply for programs and services, including government benefits and healthcare, that can support their stability and well-being.
